Case report of Successful Pregnancy of a 44 year old lady after 10 year trials and 10 Failed IVFs

"A 44 year old lady visited our fertility center with her 52 year old husband with 10 year infertility medical record at September 2016. The lady was submitted in laparoscopic surgery for endometriosis at 2007, and at 2010 she performed laparoscopic surgery and removed the right salpinx and a year later the left one was removed respectively.

The couple has started attempts for a baby since 2008 and they had their IVFs from 2011 until May 2016, and they never had a successful pregnancy. So, they heard about our center and wanted to exhaust their last chance so they would not let down their dream to have a baby.

Their next step according to our guidance was to do our specific individualized immune tests. The lady was tested for NK panel (total, endometrial NK cells, and their activity), where the endometrial NK number was too high. Also, Platelet-leucocyte-aggregates (PLA) test was performed and this marker was high too. We explained the couple that NK was the major reason for their failed implantation, and we planned therapeutic strategy in order to regulate the NK cells and go to next IVF trial with much higher chances for successful pregnancy. On the other hand, Husband’s sperm analysis and DNA fragmentation were fine, while the levels of Vitamin D3 of the lady were low and we administered supplement of Vitamin D3 and baby aspirin.

Based on the diagnostic result, 2 dosages of intralipid were administered before the embryo transfer in order to regulate suitably the NK cells for the next IVF trial. Then, the process of embryo transfer of 2 embryos followed and after 10 days a positive pregnancy test was detected for the FIRST TIME after so many years. Then a final dosage of intralipid was administered, so the danger of miscarriage can be limited down at the first trimester of gestation. At the first UV one embryo was detected with positive heart beats and pulses, and the couple was very happy. Later, The Nuchar translucency screening was followed and level 2 ultrasound by Dr. Spyros Papageorgiou and the pregnancy continued normally without any problems. The baby was born at November 2017 with 3100gr weight and the couple was very happy after all these 10 years of trials..."

Case Report of Successful Pregnancy of 35 year old lady with Autoimmune Disease after 4 RSAs

"Α 35 year old woman with her husband came to our center with her husband while she had 4 rapid spontaneous abortions (RSAs) the last 2 years and she was seeking the reasons of losing her embryos in  the 5th(two), in the 6th(one), and in the 7th(one) week of gestation. The couple already had a girl 3 years old, and after the first birth, she lady suffered from an autoimmune disease and she was under drug therapy with PLAQUENIL.

The first thing we had to clarify was to investigate the woman’s quality of eggs. So, initially AMH was measured, which was in good levels, so we didn’t have to measure  inhibin B for the quality of her genetic material.

The second thing we had to do is clarify that husband’s (37 years old) sperm was ok. We ran a sperm analysis, and DNA fragmentation, which was 6% and means excellent quality.  Also, we tested woman’s TSH and Vitamin D3, which were normal.

So, our investigation was focused in woman’s immune system.  We measured total NK cells, endometrial NK cells, NK activity, Embyotoxicity (ETA), and platelet leucocyte aggregates (PLA, special thrombophilia biomarker).  The results showed that levels of endometrial NK cells were very high (48,6%), three times more than the normal ranges, while the NK activity was slightly increased, and therefore the pregnancy results would be blocked for sure. ETA was negative, while PLA was 3 times higher than the normal ranges. The levels of endometrial NK cells were one of the highest that we have seen until now in laboratorial level, and we concluded that it was the major cause for the 4 RSAs. Our center administered targeted intravenous infusion of 20% intralipid depending on the diagnosis..."

Case of a 39 year old woman experiencing successful pregnancy without IVF after suppression of the NK cytotoxic activity

"Nowadays, there is statistic evidence worldwide that more than 10% of couples are infertile. Some times, there is no evidence for the couple’s infertility and many couples proceed to the in vitro fertilization (IVF) (Nelson and Lawlor 2011). Up to 3 % annual births in developed countries occurred because of assisted reproductive techonologies (ARTs) (Gosden et al., 2003). Most women at their 40 years of their age and over are seeking fertility through ARTs and specifically IVF (Smajdor A. 2011).

In our case, 39 year old woman was trying intensively to be pregnant the last 2 years without success. The last 9 months started asking for some tests to figure out what was the problem. The investigation started with the husband’s sperm excluding that he was infertile. The husband’s sperm was detected for motility, biochemistry and microbiology markers and finally DNA fragmentation index. All tests for the condition of the sperm were fine, which means that the husband is eligible in contributing positively in fertilization.

Then the parameters that were tested for the woman were any anatomical abnormalities in endometrium and uterus and the adequacy of primary follicles during the third day of the cycle. Then the hormonal levels were tested (LH, FSH, PRL, E2, TESTO, DHEA-S, SHBG, AMH), anti-thyroid antibodies (ATA), anti-thyroglobulin antibodies (TAT), and anti-microsomal antibodies (MAT). All serum levels of the previous tests were normal and the condition of that particular woman was considered fine. Then, a first attempt was sperm insemination was performed at the beginning of December without pregnancy success..."

"A couple (woman 51 years old and husband 56 years old) came to our center at 2013 with intense phycological and emotional stress because of the result of a failed IVFs trial at 2012. Their desire to get at least a child was too obvious. The woman was at menopause three years ago without health problem, while the husband was quite health with good sperm quality. The diagnostic testing was done to detect the reasons for the failed IVF was Ultrasound and complete gynecological check up, which did not show any pathological findings. So the diagnostic blood tests discussed were Natural Killer Cells levels and their activity and embryotoxicity assay (ETA), where our team explained fully about the potential and the advantages of these tests in the diagnosis as well as in the therapy.

 The results showed that the subpopulatin of NK cells in the area of endometrium was increased while the NK activity and ETA were at normal values..."
