New clues to restoring fertility in women with disabling ovary disorder

Findings from the research, conducted in a pre-clinical model and just published in the Journal of Clinical Investigation Insight, show that blocking androgen actions could help re-set reproductive function to normal levels by modifying brain circuitry important to fertility.

Associate Professor Rebecca Campbell from the University's Centre for Neuroendocrinology and Department of Physiology, says she and her co-investigators, Ph.D. student Mauro Silva and Assistant Research Fellow Mel Prescott, are excited about their findings which are potentially significant for women suffering from the syndrome.

"Our findings suggest that despite the early development of brain pathology in some forms of polycystic ovary syndrome, normal reproductive function can potentially be restored in adult women with the disorder through modifying the wiring in the brain," Associate Professor Campbell explains.

There is growing evidence the brain is involved in both the development and pathology of polycystic ovary syndrome. The Otago researchers' earlier work in a preclinical model on the syndrome has identified changes in specific brain circuits that may underlie the disorder.

In this most recent study, Associate Professor Campbell and her team investigated when these circuit abnormalities develop and whether the circuits are "hard-wired" or can be changed by blocking androgen actions once the disorder is established.

"We discovered that brain changes occur prior to the onset of puberty, which is before the syndrome appears, suggesting that the brain pathology precedes disease development," Associate Professor Campbell says.

"We also discovered that, despite this early 'programming' of neural circuitry, long-term blockade of androgen actions was able to completely restore normal brain wiring and reproductive cycles."

Associate Professor Campbell says that although the work is still pre-clinical, it gives clues about potentially effective therapies in treating the reproductive pathology of polycystic ovary syndrome in women.

Her research team has established a collaboration with clinicians in Sweden to retrospectively look at the impact of androgen receptor blocker drugs on long-term reproductive outcomes in women with the syndrome.

Vitamin D may be key for pregnant women with polycystic ovary syndrome

"Vitamin D may play a key role in helping some women seeking treatment for polycystic ovary syndrome (PCOS)-related infertility get pregnant. PCOS is a hormonal disorder affecting 5 to 10 percent of women of reproductive age. Left untreated, the condition can lead to long-term complications such as type 2 diabetes, elevated cholesterol, and infertility due to lack of ovulation.

Results of the new study, led by researchers at the Perelman School of Medicine at the University of Pennsylvania, showed women who were Vitamin D deficient when starting fertility treatments were 40 percent less likely to achieve a pregnancy. The results were presented this week at the annual American Society for Reproductive Medicine Scientific Congress & Expo in San Antonio, Texas.

"Traditionally, Vitamin D is a concern because of its impact on calcium absorption and bone integrity, but the last several years have seen a surge in research exploring how other biological processes -- like fertility -- are impacted by a dearth of this critical nutrient," said lead author Samantha Butts, MD, MSCE, an associate professor of Obstetrics & Gynecology at the Perelman School of Medicine at the University of Pennsylvania. "Our study builds on existing research linking vitamin D deficiency and diminished ovulation in response to fertility medications, and diminished likelihood of achieving a pregnancy that results in delivery of a live born infant showing it plays a significant role in fertility of women with PCOS."

In the study, Butts and colleagues analyzed and compared the results of two large-scale clinical trials (PPCOS II and AMIGOS), which examined the effectiveness of fertility drugs on improving pregnancy rates among women with PCOS and Unexplained Infertility respectively. Data from more than 1,000 participants revealed that regardless of body mass index, race, age, markers of metabolic functioning, or fertility treatment, vitamin D deficiency was associated with a reduced likelihood of these women becoming pregnant and delivering babies if PCOS was the underlying cause of infertility. A relationship between vitamin D deficiency and fertility treatment outcomes was not seen in the subjects from the AMIGOS trial who had Unexplained Infertility..."

"Scientists led by the University of Birmingham have discovered that a new class of male sex hormones known as androgens plays a key role in the development of polycystic ovary syndrome (PCOS).

The research, published online in the Journal of Clinical Endocrinology and Metabolism, shows that these novel androgens make up more than half of the androgen pool in women with polycystic ovary syndrome.

A common condition, believed to affect at least one in ten women in the UK, PCOS has significant impact on the life of affected women, causing symptoms which may include the following:

· Irregular periods: in which the ovaries do not regularly release eggs (PCOS is the most common cause of infertility in the UK)
· Polycystic ovaries -- in which the ovaries enlarge as more and more follicles develop but fail to release a mature egg
· High levels of androgens: male sex hormones such as testosterone, which may cause physical signs such as excess facial or body hair

While previous research exclusively focused on the role of the classic androgen, testosterone, in PCOS, this research breaks new ground by showing that a novel class of androgens, known as 11-oxygenated C19 steroids, is the major contributor to androgen excess in women with PCOS..."

Research: Some women with PCOS may have adrenal disorder

"A subgroup of women with polycystic ovary syndrome (PCOS), a leading cause of infertility, may produce excess adrenal hormones, according to an early study by researchers at the National Institutes of Health and other institutions.

PCOS is a group of symptoms related to high levels of hormones known as androgens. In many women with the condition, the ovaries contain numerous small, cyst-like sacs. Women with PCOS may have irregular, missing, or prolonged menstrual periods, excessive facial and body hair, insulin resistance, and problems with fertility. Treatment may include drugs that block androgens, and oral contraceptives, which contain the hormones estrogen and progesterone.

"Traditionally, treatment for PCOS has included modifying ovarian hormones," said Constantine Stratakis, M.D., director of intramural research at NIH's Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) and senior author of the study. "Our findings indicate that a subgroup of patients could conceivably benefit from modification of adrenal hormones as well."

The study was published online in the Journal of Clinical Endocrinology and Metabolism. The study's first author is Evgenia Gourgari, M.D., of Georgetown University, Washington, D.C., who was a research fellow at NICHD when the study was conducted.

In their article, the researchers noted that earlier studies had found that in some women with PCOS, androgens were produced by the adrenal glands, rather than by the ovaries. On the basis of these observations, the researchers decided to test the adrenal functioning of women with PCOS..."

"Weight loss and exercise improve ovulation in women who have polycystic ovary syndrome, a common hormone disorder that often causes infertility, according to a new study published in the Endocrine Society's Journal of Clinical Endocrinology & Metabolism. Polycystic ovary syndrome (PCOS) is the most common cause of female infertility. 

The condition occurs when a woman's body produces slightly higher than normal amounts of testosterone and other androgens, sex hormones associated with male traits. The resulting hormone imbalance can cause irregular or absent menstrual periods, weight gain, acne, excess hair on the face and body, or thinning hair on the scalp. As many as 5 million women nationwide have PCOS, according to the U.S. Department of Health and Human Services' Office of Women's Health.

"The findings confirm what we have long suspected -- that exercise and a healthy diet can improve fertility in women who have PCOS," said one of the study's authors, Richard S. Legro, MD, Vice Chair of Research and Professor of Obstetrics and Gynecology and Public Health Sciences at Penn State College of Medicine. "Making preconception lifestyle changes is beneficial, either alone or in combination with other pretreatment options..."
